Cialis is the brand name for tadalafil, a small yellow pill used to treat erectile dysfunction (ED) in adult men. It also treats symptoms of an enlarged prostate (BPH). The U.S. Food and Drug Administration first approved it in 2003.
Tadalafil is the active ingredient. Cialis is the brand your pharmacist may have heard of. Generic tadalafil has the same active ingredient, same dosage strength, and the same effect. The FDA approved generic versions in 2018 . The generic is what most insurance plans cover.
How It Works, Without the Jargon
When you’re aroused, blood needs to flow into two chambers inside the penis. Tadalafil relaxes the blood vessels so that blood can get in and stay there. It isn’t a hormone. It doesn’t raise your sex drive. If you’re not aroused, it doesn’t do much of anything.
One thing people don’t always know: tadalafil stays in your blood for about 17 hours. Compare that to sildenafil (Viagra), which leaves your system in roughly 4 hours. That’s why Cialis is nicknamed “the weekend pill” — one dose on Friday can still work on Sunday morning for most men.
Dosages We Stock
The FDA approved four tablet strengths: 2.5 mg, 5 mg, 10 mg, and 20 mg. On our site, you’ll see all four listed under this category.
- As-needed: 10 mg about 30 to 60 minutes before sex. The dose can go up to 20 mg or down to 5 mg based on how it works and how you tolerate it.
- Once-daily: 2.5 mg or 5 mg taken at the same time every day, with or without sex. The once-daily version is also the dose approved for BPH symptoms.
You won’t see a one-pill-fits-all answer because there isn’t one. A 20 mg dose for one man is too much. A 5 mg daily dose for another man works for years.
Generic vs. Brand — What You’re Actually Paying For
Same factory standard, different price.
Generic tadalafil typically costs a small fraction of brand Cialis. GoodRx lists average U.S. retail around $10 to $44 per pill for generic tadalafil, versus $12 to $80 for brand Cialis depending on strength (GoodRx tadalafil pricing). SingleCare’s data puts generic tadalafil 5 mg as low as about $0.19 per tablet with their coupon (SingleCare tadalafil). We pass that spread through in our pricing tiers on this page.
If you have insurance, almost every U.S. plan covers generic tadalafil and refuses brand Cialis. If you’re paying out of pocket, generic is usually the right call.
Who Should Not Take It
This part matters. Skim it.
Do not take tadalafil if you use nitrate medicines for chest pain. Nitrates include nitroglycerin, isosorbide dinitrate, and isosorbide mononitrate. The combination can drop your blood pressure to a dangerous level. Published guidance says wait at least 48 hours after tadalafil before taking a nitrate (The Cardiology Advisor, 2023; drugs.com interaction listing). That window can be longer for higher doses or for men with kidney or liver problems.
Also avoid tadalafil if you:
- Take guanylate cyclase stimulators such as riociguat (Adempas) for pulmonary hypertension
- Have been told by a doctor to avoid sexual activity because of heart problems
- Had a heart attack or stroke within the last 6 months
- Have uncontrolled high or low blood pressure
- Have severe liver disease or are on dialysis
This is not a complete list. Talk to your doctor or pharmacist before ordering.
Side Effects Worth Knowing
Most men who take tadalafil tolerate it well. The most common side effects reported in clinical trials are:
- Headache (roughly 1 in 10 men)
- Back pain or muscle aches (usually 12 to 24 hours after the dose)
- Flushing of the face
- Stuffy or runny nose
- Indigestion
Back pain is the one that surprises people. It’s harmless in most cases, but if you’ve never heard of it, you might panic the first time. We mention it because we want you prepared, not worried. Severe side effects — sudden vision loss in one or both eyes, sudden hearing loss, an erection lasting more than 4 hours — are rare. Stop the medicine and seek medical help right away if any of those happen.
Drug Interactions Worth Flagging
Beyond nitrates, three other classes of drugs interact with tadalafil:
- Alpha-blockers (tamsulosin, alfuzosin) for prostate or blood pressure problems — your doctor may stagger the doses or pick a lower strength.
- Antifungals and certain antibiotics — ketoconazole, itraconazole, clarithromycin, and others can raise tadalafil levels in your blood.
- HIV protease inhibitors — ritonavir and similar drugs delay how fast your body clears tadalafil.
Grapefruit juice does the same thing in a smaller way. Two glasses are usually fine. A whole pitcher a day might not be.
Tell your prescriber every medicine, supplement, and recreational substance you use. Yes, even poppers (alkyl nitrites). They contain nitrites and you’re back to that 48-hour rule.

Prescriptions and What We Need
Cialis is a prescription medicine in the United States, the United Kingdom, the European Union, and most countries where we ship. We need a valid prescription before we ship a prescription-strength product. If you don’t have one, we can connect you with a licensed telehealth provider who can evaluate you online. The eval usually costs less than an urgent-care copay and takes about 15 minutes. If they prescribe, the prescription comes straight to us.
If you upload a prescription from your own doctor, please make sure it includes:
- Your full name and date of birth
- The drug name (tadalafil or Cialis)
- Strength (2.5, 5, 10, or 20 mg)
- Quantity
- Number of refills
- Prescriber’s name and DEA or NPI number where applicable
What We Did Wrong, Then Fixed
We want to be honest here.
When we first launched this category page, we copied the dosing advice from a major compounding pharmacy website because we assumed their info was correct. A customer on dutasteride flagged it within a week — dutasteride and tadalafil both need liver-function monitoring, and our copied text didn’t mention it. We pulled the page, rewrote the warnings with input from two pharmacists we work with, and rebuilt the FAQ. We check those drug-interaction notes against the FDA label every quarter now. We still miss things. When we do, we’d rather hear from you than guess.
That’s the trade we offer: we’d rather lose a sale to a customer who’s safer taking a different medicine than win one thinking the wrong pill is fine.
A Few Things People Ask Before They Order
- “Can I split a 20 mg tablet to save money?” Yes, in some cases. Tadalafil tablets are not scored, so splitting them evenly is hard without a pill cutter. If you take 10 mg most days and 20 mg rarely, ask your prescriber to write the prescription that way so you’re not guessing.
- “How long before sex should I take it?” As-needed dosing: 30 to 60 minutes before. Once-daily dosing: same time every day. The as-needed version peaks around 2 hours, but most men get a useful effect from 30 minutes out to about 36 hours later.
- “Will it make my erection last longer after climax?” No. Tadalafil helps with getting and keeping an erection. It doesn’t change your refractory period, which is the natural recovery time between erections.
- “Can I drink alcohol with it?” A drink or two is fine for most men. Heavy drinking plus tadalafil can drop blood pressure and make it harder to get an erection — the opposite of what you’re trying to do.
- “Is the 5 mg daily dose weaker than the 10 mg as-needed?” Different, not weaker. The 5 mg daily dose builds up evenly in your blood. The 10 mg as-needed spike is higher. If you have sex more than twice a week, daily dosing is usually cheaper and steadier.

Larger packs cost less per pill. We do not charge extra for prescriptions we already have on file from a prior order.
If a price looks higher than a U.S. walk-in pharmacy, it usually is. Walk-in pharmacy pricing assumes your insurance reimburses the pharmacy. Our pricing assumes no insurance. The comparison rarely makes sense unless you’re paying cash at both ends.
Safety Note We Always Include
Tadalafil is a real drug with real interactions. It is not a vitamin. Read the patient information leaflet that ships in every order. If anything in that leaflet doesn’t match what your doctor told you, call your doctor before taking the first pill.
For the official U.S. patient guidance, see the FDA label for Cialis/tadalafil. For the U.K. version, see the NHS tadalafil page. If you live somewhere else, your country’s regulator will have an equivalent document. We provide copies of the relevant leaflet with every order we ship.
